Tailored Support Plans for Achieving NDIS Goals

At Compass, our In-Home and Community Supports are delivered through a person-centred approach that ensures each individual’s needs, goals, and preferences are at the centre of their supports. We begin by developing a tailored support plan in consultation with the person, their family, and key stakeholders. This plan identifies specific NDIS goals, daily living needs, and opportunities for community participation.

Our support workers are carefully matched based on skills, experience, and personal compatibility to foster trust and consistency. They assist with a wide range of tasks, from personal care and learning how to complete household jobs eg; washing, cooking, cleaning and simple maintenance to skill development and accessing community activities. Emphasis is placed on promoting independence, dignity, and choice, ensuring individuals have control over how and when supports are provided.

Community Connections & Inclusive Support

In the community, supports are focused on building connections and inclusion. This may involve accompanying individuals to appointments, recreational activities, social groups, or employment and training opportunities. Safety, respect, and professionalism underpin every interaction, supported by continuous staff training and adherence to quality standards.

We regularly review and adjust support plans to reflect progress, changing needs, or new goals, ensuring that each person receives flexible, reliable, and outcome-focused support that empowers them to live the life they choose.
