How can our

Navigators assist you?

Support Coordination is a funded service under the National Disability Insurance Scheme (NDIS) designed to help participants understand, navigate and implement their NDIS plans. The goal of Support Coordination is to empower participants to build their capacity to manage their own supports, connect with services and achieve their goals.

Who Can Access Support Coordination?

Support Coordination is not included in every NDIS plan. It is funded based on individual circumstances, such as complexity of needs, lack of informal support networks, or difficulty in navigating the system. Participants can request Support Coordination during their planning meeting or plan review.

Types of Support Coordination

The NDIS funds three levels of Support Coordination based on a participant’s individual needs:

  • Support Coordination (Coordination of Supports) – More comprehensive assistance that helps participants strengthen their ability to coordinate their supports, build capacity and manage service providers effectively.
  • Psychosocial Recovery Coaching – A recovery coach is a worker that has mental health knowledge. A recovery coach will: 
    • spend time with you and people important to you, to get to know you and understand your needs
    • help you to find out about different services and supports and how these can help you
    • help you get support from mental health services
    • help you better understand the NDIS and support you with the NDIS

Role of a Support Coordinator

A Support Coordinator assists participants by:

  • Helping them understand their NDIS plan, including funding categories and how to use them.
  • Connecting them with service providers and community supports that align with their goals.
  • Assisting with service agreements and ensuring services are delivered effectively.
  • Supporting participants in building the skills to manage their own supports over time.
  • Resolving issues and addressing barriers that may arise in service delivery.
  • Helping participants prepare for NDIS plan reviews by reflecting on their progress and future needs.
